CareOne Senior Care has expanded dementia support in Southeast Michigan by becoming a GUIDE Respite Service Partner Organization. Through Medicare’s GUIDE Model, eligible families can access care navigation, caregiver education, 24/7 support, community resources, and up to $2,625 annually in respite care. FARMINGTON HILLS, MICH. — September 11, 2026 — CareOne Senior Care, a non-medical home care agency that has served families across Southeast Michigan since 2015, has entered new partnerships that allow it to serve as a GUIDE Respite Service Partner Organization. The designation ties CareOne into Medicare’s Guiding an Improved Dementia Experience (GUIDE) Model and adds a new option for dementia care in Farmington Hills and the surrounding area: in-home respite care for families enrolled in the program.

“Families caring for someone with dementia are often stretched thin in every direction, coordinating appointments, managing behavior changes at 2 a.m., trying to hold onto their own job and health,” said Carrie Akin, Vice President of Operations and Co-founder of CareOne Senior Care. “GUIDE gives us a real way to step in. We can put a trained caregiver in the home so a spouse or adult child can leave the house for a few hours and not spend the whole time worrying.”

GUIDE was built by the Centers for Medicare & Medicaid Services to support both the person living with dementia and the family member caring for them, and it comes with no cost share for GUIDE services. Eligible families receive the following benefits:

  • Dedicated care navigator.

  • Individualized care plan.

  • Caregiver training.

  • 24/7 support line.

  • Help lining up transportation and meals.

  • Coordination between doctors and community resources.

Participation does not require waiting for advanced stages of dementia; individuals at mild, moderate, or severe stages may qualify. Eligibility requires enrollment in Original Medicare Parts A and B and not being enrolled in Medicare Advantage, PACE, hospice, or a long-term nursing facility.

Qualifying families can receive up to $2,625 per year in respite services, and CareOne’s caregivers provide that care in the home, helping with companionship, supervision, personal care, meal preparation, and mobility support, depending on what’s authorized in the individual’s respite plan. That’s time a family caregiver can use to rest, get to their own doctor’s appointments, keep working, or just leave the house for a while, knowing someone reliable is with their loved one.
